ATTENTION SENIOR PARENTS:
This is a follow-up to the letter you should have received in mid-May, 2014.
The State Board of Health is requiring the following immunizations in order for your student to be eligible to attend the 2014-15 school year:

      
     ONE (1) BOOSTER DOSE OF MCV 4 IS REQUIRED FOR STUDENTS IN GRADE 12
     ALL OTHER REQUIREMENTS FROM THE 2013-14 SCHOOL YEAR WILL REMAIN UNCHANGED.

Students who do not have records of the MCV 4 booster on record by August 22nd, will not be allowed to attend school  (absences will be marked as unexcused or truant).

ALL STUDENTS AND PARENTS: BE SURE TO CHECK YOUR POWERSCHOOL ACCOUNTS FOR ACCESSING PURPOSES.  REPORT ANY PROBLEMS TO THE HIGH SCHOOL OFFICE IMMEDIATELY FOR CORRECTIONS.
